Referrals:

Whether you have a chronic health condition, disability, impaired vision, dementia, or are finding daily tasks more challenging with age, we offer comprehensive occupational therapy services tailored to meet your needs.

I welcome referrals from GPs, specialists, aged care providers, support coordinators, case managers, and private individuals. Whether you are seeking a one-off assessment or ongoing support, we work collaboratively to ensure timely, high-quality service delivery.

Information from DVA regarding OT services:

Who can receive it
You may be able to receive OT if you have an assessed clinical need and a:

Veteran Gold Card; or 
Veteran White Card and the treatment is for a condition covered by your card

 

What you can receive
If you are eligible for OT, your therapist may:

  • work out a treatment plan with you to suit your needs

  • help you adjust your home or work setting to your needs

  • help you improve your mental and physical skills; and

  • provide you with aids to make your tasks easier


How you access

 

  1. Get a referral to an OT from (D0904 - DVA Referral):
    your general practitioner (GP), a medical specialistyour treating doctor in hospital; or a hospital discharge planner

  2. Contact the OT to make an appointment. Confirm they will accept your Veteran Gold Card or Veteran White Card for their services.

About My Aged Care:

My Aged Care is a government-funded program designed to support older Australians in accessing aged care services and support to help them live independently and stay connected to their communities.

 

Occupational Therapy (OT) plays a crucial role within My Aged Care by providing tailored interventions and support to enhance the well-being and independence of older adults.

 

OT services are integrated into various streams of My Aged Care funding, including:

  • Home Care Packages (HCP)

  • Commonwealth Home Support Program (CHSP)

  • Short Term Restorative Care (STRC) Program

Our My Aged Care OT Services:

1. OT Home Visit
We provide personalised home visit occupational therapy services, delivering assessments and interventions directly in the comfort of your own home. Our therapists work collaboratively with you to identify goals, address challenges, and develop customised plans to support your independence and well-being.

2. Minor/Complex Home Modifications
Judy-Ann specialises in conducting assessments and providing recommendations for minor and complex home modifications to improve safety, accessibility, and functionality within your home environment. Our goal is to create a living space that promotes independence and supports your unique needs and preferences.

3. ​Assistive Technology
Judy-Ann can provide expert guidance and comprehensive support throughout the entire process of selecting and implementing assistive technology solutions for older adults. With a focus on enhancing mobility, safety, independence and overall quality of life, we work closely with individuals to understand their unique needs and preferences.

Private Clients

We also accept referrals from:
